The LPN provides direct nursing care, under the direction of the Home Care Nurse/Assessor/Manager (HCN/A/M) RN.
Duties Relating to Program Delivery
- Receives assignments from the HCN/A/M and demonstrates an understanding of the assignment
- Performs nursing skill required to meeting the assigned task in a safe, competent manner
- Reports any changes in client condition/situation to the HCN/A/M
- Documents the results of the assigned task
- Involves client/family in performing tasks, whenever possible
- Recommends to HCN/A/M revisions to the care plan where appropriate
Duties Relating to Accountability
The LPN is accountable for reporting, recording, working within program guidelines and managing the workload of the assigned tasks. Duties in this area may include:
- Reporting any concerns about the client safety or health risks to the HCN/A/M
- Reporting any changes in the client’s condition/situation
- Participating in case conferences
- Carrying out assigned tasks
- Maintaining confidentiality
- Reinforcing program mission statement and philosophy to client, family and community
- Notifies HCN/A/M when unable to complete a required task
- Submitting monthly statistics/reports as required
Qualifications
- Graduate of an accredited school for Licensed Practical Nursing
- Current license to practice and in good standing with SALPN
- A minimum of one year experience
- Knowledge/Awareness of the First Nations language/culture is an asset
- Maintain a valid Saskatchewan Driver’s license
- Demonstrated ability to work with others and take direction
- Maintain First Aid and CPR certificates
- Previous experience working with First Nations is an asset
- Must provide own transportations for work purposes
